Individual Case Worker (ICW)
PoCoMo has been gifted with an annonymous donation that has assisted us in providing one to one support to those youth who need extra support.
The ICW will be receiving primary referrals from Project Reach Out-Tri-Cities, as well as from community parteners and self referrals.
ICW Program Description
Individual Case Worker is a referral program responsible for providing comprehensive one to one counselling services for youth ages 12-18 years of age, and to help improve the well-being and functioning of these youth.
PoCoMo’s provides an Individual Case Worker who is a generalist and works with a wide range of youth. They engage youth in developing a goal plan that meets their particular needs and accomplish specific goals. They focus on improving individual skills and connections with community resources and supports.
The case worker provides assistance with four domains of the youth’s life: individual, peer, school, and community. The case worker engages and actively supports youth who are at risk.
The case worker can assist youth in areas such as improving their relationships, addressing personal problems, schools concerns, drug/alcohol issues, friend/peer pressure, and learning new life skills.
Some of the ways the case worker may assist the youth are by increasing communication skills, managing anger, reducing conflict, and establishing effective support systems.

REFERRAL STRUCTURE
The ICM worker primarily accepts referrals from PoCoMo’s Project Reach Out program. In addition to, referrals are accepted from other community resources (e.g schools, MCFD, Park and Recreation) as well as self-referrals. Each referral is assessed on an individual bases to determine needs and the appropriate response (intake) or referral.
ASSESSMENTS
The Case Manger will implement a risk and needs assessment of clients to determine the appropriate response, and/or referral. The assessment forms will be used to track the assessment process, as well as assist in determining needs and goal plans.
INTAKE
If the Case Manager determines that a client intake is appropriate, a client file will be opened and maintained.
